You will be supporting clients and families within aged care settings across community and residential services.
Responsibilities may include:
- Conducting assessments with clients and families
- Facilitating SAT, EPA and EPG applications
- Completing financial applications
- Assisting with accommodation applications and aged care placements
- Liaising with families, multidisciplinary teams and external providers
- Supporting clients through aged care transition processes
- Bachelor of Social Work
- Relevant Social Work qualification and eligibility for membership
- Experience within aged care social work and/or NDIS settings
- Strong understanding of aged care processes within WA
- Experience conducting client assessments
- Excellent communication and interpersonal skills
- Ability to work independently within fast-paced environments
