This is a rewarding opportunity to provide essential healthcare services to regional Australian communities.
The Role:
- Working with older, aged community and people with a disability to maximise their independence at home
- Liaising with clients, carers and families to provide practical solutions to promote independence and safety at home
- Developing goals and care plans to assist patients to remain at home and active within their local community
- Collaborating with other clinicians to offer holistic care to clients referred
- Working with a diverse range of patients and health conditions.
- Working across funding streams such as HACC PYP, CHSP & possibly the minimal NDIS work
- Opportunity to work with Allied Health Assistants to undertake client care programs and equipment trials
- Primarily home-based work with some opportunity for clinic appointments
About You:
- Community OT experience
- Knowledge of Australian Building Standards with Home modifications
- Experience with Scooter assessments, Wheelchair prescription
- Strong communication and interpersonal skills with a pleasant, kind manner
- Current, valid AHPRA Registrations
- Have a valid Background/ Police Check, plus are able to provide at least 2 working references
- Be comfortable to work in regional setting
- Familiar with funding schemes such as Geat2GO and SWEP is desirable
- Working with the elderly is a bonus!
