In this role, you’ll:
- Manage an adult community caseload across Support at Home, CHSP and NDIS (16+)
- Complete comprehensive functional and environmental assessments
- Lead complex home modification assessments and recommendations
- Deliver detailed assistive technology and equipment prescriptions
- Conduct 2–3 home visits per day with structured documentation time
- Prepare high-quality, evidence-based reports
- Mentor and supervise early-career clinicians
- Contribute to quality improvement and best-practice service delivery

Who this role is ideal for

- Have strong experience in community-based adult OT
- Are confident in home modifications and assistive technology
- Enjoy structured billable targets with operational support
- Like autonomy in your day while maintaining MDT connection
- Want to mentor junior clinicians
- Are comfortable with a community-based schedule
- May be open to splitting time between West Melbourne and Geelong
What you’ll bring
- Occupational Therapy qualification
- Current AHPRA registration
- Experience in community OT (NDIS / aged care / Support at Home)
- Strong report writing skills (home mods & AT experience highly regarded)
- Driver’s licence and access to a vehicle
- Excellent time management and organisational skills
