We are currently partnering with a well-established, values-driven aged care provider to recruit an experienced and compassionate Social Worker. This is a full-time position, Monday to Friday.

The Role
In this role, you will play a key part in supporting the psychosocial wellbeing of residents and their families. You will provide tailored guidance, advocacy, and care coordination, ensuring individuals are supported through care planning, transitions, and decision-making processes.
Working closely within a multidisciplinary team, you will contribute to delivering holistic, high-quality care outcomes while empowering residents to achieve their personal goals.

Essential Criteria
- Bachelor of Social Work (or equivalent qualification)
- Experience aged care - Highly desired
- Strong organisational and time management skills
- Excellent communication and interpersonal abilities
- Sound problem-solving and negotiation skills
- High attention to detail
- Ability to work both autonomously and within a team
