About the Transition Care Program (TCP):
They are the lead allied health service provider for the program within the Townsville Hospital and Health Service (THHS).
Established in 2005, the TCP is a joint Commonwealth and State-funded initiative designed to support older Australians in regaining physical and psychosocial function following a hospital stay. Delivered nationally through State and Territory Health Departments (Approved Providers under the Aged Care Act 1997), the program offers short-term restorative care, up to 12 weeks, aimed at enhancing independence and avoiding premature entry into residential aged care.
The program is goal-oriented, time-limited, and therapy-focused, with services tailored to each individual. These may include nursing support, personal care, domestic assistance, transport, and low intensity allied health therapies such as Physiotherapy, Occupational Therapy, Speech Pathology, Dietetics, and Social Work.
In our region, the THHS TCP offers 46 community-based places, servicing Townsville, Magnetic Island, Ingham, Charters Towers, Ayr, Home Hill, and Mt Isa.
