In this role, you'll work across a hybrid model(teleassessments + site visits) conducting detailed clinical assessments and developing personalised care plans. Your role is central to supporting both client outcomes and clinical quality across aged care and disability services.

- Conduct clinical assessments for clients under HCP, CHSP, NDIS, and SIL programs
- Collaborate with case managers and allied health professionals to develop and monitor care plans
- Participate in clinical governance, audits, and continuous improvement initiatives
- Support onboarding, training, and supervision of support staff
- Liaise with families, GPs, and external providers to ensure continuity of care
- Registered Nurse (Division 1) with AHPRA
- 4–5 years’ experience in aged care and/or disability
- Strong background in clinical assessment and care governance
- Skilled communicator with accurate documentation skills
- Comfortable with a hybrid work model(in-person + telehealth)
- Experience using My Aged Care, PRODA, or NDIS portals
- Involvement in clinical quality improvement projects or audits
